You will deliver comprehensive mental health assessments, develop personalised care plans, and implement home-based interventions, supporting the diversion of low to medium acuity cases from hospital Emergency Departments. This role is central to providing safe, least restrictive care that enhances outcomes for consumers while supporting the broader community.In this position, you will provide expert clinical nursing care and leadership, guiding the team and coordinating projects that improve service delivery and consumer outcomes. You will work within national mental health frameworks, delivering education, counselling, and group work, while consulting with other health professionals and community organisations.
