Our Disability and Specialised Services (DaSS) Division delivers essential support to South Australians through high-quality NDIS services, sector reform, inclusion-focused policy. We provide direct disability supports through Supported Independent Living, and specialised care through the Exceptional Needs Unit, Clinical Services, the Equipment Program and Northgate Aged Care services. Our work is strengthened by Corporate and Engagement teams, ensuring strong governance, quality, and positive outcomes for people we support. Role Details The Practitioner, Specialist Family Support Pathways provides: Early intervention support to keep families together with a focus on children with complex disabilities who access the National Disability Insurance Scheme (NDIS). Supports and assists individuals with disabilities who do not reside in traditional care settings. Undertakes clinical assessments for suitability of support services; and case management of individual and family situations. Special Conditions National Police Check required.Employment-related Screening Check required.Some out of hours work may be required.Some interstate and intrastate travel may be required.Must hold a current Australian issued drivers licence (equivalent to minimum class C).Duration: Term up to twelve (12) months Essential Qualifications Incumbent should hold an appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers; or an appropriate degree or equivalent in Bachelor of Disability and Developmental Education, which enables eligibility for full membership of Developmental Educators Australia Incorporated; or an appropriate degree or equivalent qualification in Disability and Community Rehabilitation; or an appropriate degree or equivalent qualification in Psychology and registration with the Australian Health Practitioner Regulation Agency; or an appropriate degree or equivalent qualification which entitles registration as an Occupational Therapist with Occupational Therapy Australia.
