The Opportunity Were seeking a passionate Occupational Therapist to join our Hobart team (part time 4 days a week), where youll empower clients to live independently and achieve their goals through personalised, community-based care. Youll work across a variety of programs including HCP & CHSP as well as PHI and privately funded patients. What Youll Do Provide client-centred services such as home assessments, equipment prescription, and home modifications Support clients post-hospital discharge to enhance safety and reduce the risk of readmission Additionally for residential aged care clients you will develop and run group and individual programs Prepare clear, practical reports that directly support care deliverywithout the burden of traditional bureaucratic documentation AHPRA registered (Full or Limited Remote Level) New grads welcome; experience in community or aged care is a plus Skilled in promoting health and wellbeing through client-centred care Comfortable working independently and travelling across Hobart (vehicle required; travel reimbursement provided) Not based in Hobart?
