What is the opportunity? In this critical role, you will support young people living with complex and persistent mental health conditionsoften combined with other complexities such as trauma and cognitive impairment. Youll work closely with clients, their families, and a multidisciplinary team to deliver recoveryoriented, personcentered care that enhances quality of life and independence. Key Responsibilities Provide individualised, traumainformed support in line with clients care and recovery plans Deescalate behaviours of concern and respond effectively in crisis situations Assist with daily living activities, community participation, and skill development Support clients in managing mental health symptoms, medication adherence, and health appointments Foster meaningful relationships and promote social and emotional wellbeing Maintain accurate, professional documentation and contribute to team care planning Maintaining accurate health care documentation as required What We Require From You Minimum 2 years experience working in youth or disability support, with a focus on complex client needs Proven experience supporting individuals with dual diagnosis, trauma history, or psychosocial disabilities Strong understanding of recoverybased, personcentered, and traumainformed care principles Exceptional communication, conflict resolution, and risk management skills Ability to work independently and as part of a multidisciplinary team Current First Aid & CPR and NDIS Worker Screening Check Recent (within the past 12 months) police check Valid drivers license and access to a reliable, insured vehicle We Also Highly Regard Candidates Who Hold a Certificate IV or higher in Mental Health, Community Services, Disability, or equivalent qualification Are trained in positive behaviour support and implementing behaviour support plan Sirv is a leading Disability Support Provider that operates nationally.
