Our participants may have a mental health condition, Acquired/Traumatic Brain Injury (A/TBI), or disability resulting from a workplace or traffic incident.Your role will be to help them build the skills and confidence to manage daily tasks independently, with a strong focus on empowerment over direct intervention. The role operates in a hybrid model, combining work-from-home, office (Bendigo site), and outreach support. This program operates with a large catchment area in Bendigo and the surrounding regions.This role entails:Community outreach and telehealth support, case management and following established behavioural support plansRegular communication with care teams, linking participants to a wide range of community and clinical servicesDeveloping professional relationships with individuals, their families and others stakeholders in the communityManaging referrals, performing risk assessments, providing accurate and timely case notes and reportsUse of a variety of systems and navigate multiple platformsThis is a full-time Parental Leave cover position, Monday to Friday, 9am-5pm.About ermha365 and what we can offer youAt ermha365 we work with clients with some of the most complex mental health and disability support needs in Victoria.
