Job Description About the Role We are looking for a Wellbeing Clinician to facilitate a social prescribing pilot program in Gawler Were growing our services and looking for dynamic, values-driven individuals to contribute to our innovation and expansion. Your Key Responsibilities: - Provide non-clinical, person-centred social prescribing support through solution-focused appointments within participating general practices - Conduct holistic strengths-based assessments exploring participants social, emotional, cultural, and wellbeing needs, including experiences of social isolation and loneliness - Conduct risk assessments and make referrals to specialist services as required for clients with complexneeds that may be outside the scope for social prescribing or require specialist intervention Whats in It for You?
