Co-Facilitator/Case Manager - FDV Team - Aboriginal Throughcare This role is a dual position within Outcares Reintegration Team, combining group facilitation of a Brief Intervention Mens Behaviour Change Program in a mens prison with individual case management. The responsibilities include fostering a safe, accountable environment and managing the progress of participants in addressing family violencerelated behaviours. Key responsibilities Cofacilitate the brief intervention mens behaviour change group in line with programme guidelines and best practice standards. Create a safe yet challenging environment that encourages participant accountability. Apply culturally responsive and traumainformed approaches. Plan, coordinate and implement codesigned, personcentred case plans for clients in accordance with Outcares contractual and legislative policies and protocols, ensuring attainable goals and appropriate timeframes. Conduct assessments of allocated clients and liaise with service providers as appropriate, within the community or in a prison setting, providing structured social support for clients within the justice system. Communicate and interact with clients and their families to ensure appropriate intervention models and strategies meet client needs, while meeting all nondiscriminatory and equality standards. Provide monthly reporting to the Team Leader and ensure data accuracy as per reporting requirements. Maintain documentation, logs, reports and records of client interactions and services provided in appropriate files and databases. What you bring Relevant tertiary qualification in Social Work, Psychology, Criminology, Human Services or Community Services is highly regarded; applicants with demonstrated relevant experience are also encouraged to apply. Current working rights in Australia. Valid WA C class drivers licence and willingness to travel/use a vehicle (if required) in the metropolitan/surrounding region. Current Working with Children Card (or equivalent) and a satisfactory National Police (Criminal History) check. Experience facilitating mens behaviour change groups. Demonstrated ability to work within custodial settings. Sound knowledge of community practice standards, community resources, and ability to develop personcentred client action plans.
