About The Role As the Client Support Partner, you will be responsible for working with new and existing clients who are living with a disability to support them by the development and implementation of care plans, participant goal setting, and reablement strategies to ensure that our clients receive quality, person-centered care that enhances their independence and wellbeing and promotes their positive outcomes. Your Responsibilities Developing, reviewing, and maintaining individualised care plans that reflect participant goals, preferences, and funding eligibility. Working closely with clients, families, and support networks to ensure services align with reablement and wellness principles. Conducting quarterly budget reviews and ensuring services remain within allocated budgets. Undertake regular reviews of service agreements. Performing risk assessments related to clients health, home environment, and personal safety. Assisting participants in understanding their approved services, funding classifications, and rights within the relevant framework. Promote HenderCare services to potential clients and referral sources to increase service capacity. Ideal Candidate Relevant experience in NDIS Case Management or similar Knowledge of relevant funding models or desire to learn Strong understanding of the wellness and reablement approach and its application. Experience working with vulnerable and diverse client groups, including those with dementia, disability, or mental health challenges. Certificate IV in Aged Care, Disability or Community Services (Desirable) Only applicants with the right to work in Australia will be considered.
