The role supports patients across the preoperative and postoperative continuum, providing assessment, education, and guidance to ensure safe and effective recovery. In addition, you will also manage patients within the Chronic Disease Management Program, overseeing the development and coordination of personalised care plans. You will provide patient education and coaching, supporting selfmanagement to achieve optimal longterm health outcomes.
