About the Opportunity The Social Programs Team Leader leads and manages a centre-based respite service, providing quality care and support to ageing clients, including those living with varying stages of cognitive impairment. This is a site-based, handson role that goes beyond administration. It involves active participation in the day-to-day running of the program and regular social interaction with clients, fostering engagement, connection, and a sense of community. The Team Leader is expected to lead by example, demonstrating personcentred values and bestpractice approaches in client engagement and care delivery. The position covers comprehensive day program operations, including advocacy, facilitating client referrals and admissions, conducting assessments and care planning that reflect individual needs and preferences, and maintaining an emphasis on enablement and meaningful participation. With strong leadership capabilities, you will guide staff development, manage referral pathways with internal and external stakeholders, and uphold operational standards while driving continuous improvement and delivering exceptional service outcomes. About You You will have comprehensive experience and understanding of personcentred care principles and behavioural management techniques within dementia care and cognitive impairment across the agedcare or disability sectors. You will possess mature leadership and management skills with demonstrated experience in supervising teams and coordinating complex operational activities within healthcare or aged care environments. Additionally, you will demonstrate specialised knowledge of assessment processes, care planning, and therapeutic interventions that promote enablement and meaningful participation for our clients. Diploma in Nursing, Aged Care, Disability or Community Services. Minimum 2 years in Aged Care, specifically in Dementia Care.
