Community Occupational Therapist | Aged Care Permanent Position: Part Time or Full Time (Monday to Friday) About the Role We are seeking a passionate and dedicated Occupational Therapist to join our Community team. Reporting to the Community Clinical Services Lead, you will help improve the health and wellbeing of clients receiving Aged Care funding in a community setting. Key Responsibilities Complete function and needs assessments based on subjective and objective information. Contribute to goaldirected care planning. Develop and implement tailored intervention plans to enhance safety, mobility and daily living skills. Recommend and prescribe assistive technology, adaptive equipment, and home modifications. Deliver services under eligible funding streams, including CHSP and Support at Home, ensuring accurate documentation and billing compliance. Provide education and training to clients, carers, and support staff on safe and effective strategies for daily living. Collaborate with GPs, allied health professionals, care coordinators, and family members to ensure holistic care. Maintain accurate and timely documentation through forms and online notes. Generate and process referrals as required both internally and externally. Active participation in Cape Care Community team meetings. About You Key Requirements Tertiary Qualifications Bachelor of Science (Occupational Therapy). Current registration with AHPRA as an Occupational Therapist. Minimum of 2 years in a similar role within aged care (highly desirable). Sound understanding of people living with dementia and/or complex care needs (highly desirable). Passion for working with the elderly. Commitment to a personcentred approach to care interventions. Sound clinical assessment and documentation skills.
