The Role We are seeking a dedicated Case Manager to join the Home: Noffs Youth Housing Service to support young people facing homelessness. This program works with personcentered, traumainformed and strengthsbased approaches to support young people experiencing homelessness. Home focuses on early intervention for those at risk of homelessness by providing intensive support to rapidly and safely rehouse young people in crisis, building independence to maintain stable housing and developing support networks to address complex support needs. Responsibilities Provide support to young people with complex needs such as substance misuse, mental ill-health, trauma histories, child protection/Out Of Home Care involvement, and engagement with the criminal justice system. Conduct thorough assessments to formulate clinical and case work treatment plans. Undertake outreach crisis support to young people that are unhoused. Develop detailed and holistic case plans with clients to identify housing and other goals (e.g. education, health, social, financial). Provide case management and living skills support to clients housed in transitional housing programmes and the Rent Choice Youth programme. Assist clients to secure longterm stable accommodation such as social housing and private rental accommodation. Provide brokerage and material aid support where necessary. Liaise effectively with referrers, Home team members, specialist services, government organisations, key stakeholders and/or family and carers to ensure the best service for our young people. Complete followup assessments with clients to monitor progress. Attend and/or facilitate programmes and community events for young people. Maintain accurate and uptodate client records, statistics, and reports. Qualifications & Requirements Relevant tertiary qualifications (e.g. Social Work, Psychology, Community Services, Youth Work). Minimum 2 years experience working in the homelessness sector. Demonstrated experience supporting young people with complex needs. Prepared to commit to the Ted Noffs Foundations philosophy and approach. Successful candidates must have a current and valid NSW Working With Children Check (WWCC) before commencing employment. Current NSW Drivers Licence (minimum provisional licence) and a good driving record. Demonstrated knowledge and experience working with young people, particularly those with homelessness, AOD and mental health issues. Minimum 2 years case management experience.
