The opportunity Were seeking an Occupational Therapist to join our Amplar Health Home Health Allied Health team in Bunbury, delivering highquality, communitybased care to clients supported through Support at Home programs. This is a permanent role with genuine flexibility, offering 0.4 FTE through to fulltime. Whether youre looking for a few days a week or fulltime hours, were open to tailoring the role around the days and hours that work best for you. Youll manage a Support at Home caseload, delivering clientcentred OT services in peoples homes and community settings across Bunbury and surrounding areas. What youll be doing Delivering clientcentred occupational therapy services to Support at Home clients Completing home and environmental assessments, equipment prescription and home modifications Supporting falls prevention, reablement, wellbeing and functional independence Developing care plans aligned to funding and service delivery guidelines Maintaining highquality clinical documentation, reports and progress notes Working collaboratively within a multidisciplinary allied health team Delivering facetoface care, with telehealth used where clinically appropriate About you Youre a compassionate, organised and motivated Occupational Therapist who enjoys working in the community and making a genuine difference.
