Service Manager Transition Care Program (TCP) Brightwater Kingsley The opportunity Were looking for an experienced Service Manager to lead Brightwater Kingsleya 41bed Transition Care Program (TCP) service. This is a handson leadership role in a fastpaced, shortstay environment where youll partner closely with a multidisciplinary team to help older people recover after hospital, build confidence, and transition safely back home (or into the right longerterm option). TCP provides shortterm accommodation and reablement support for older people directly after discharge from hospital. Youll ensure comprehensive assessment and goal setting, coordinate the delivery of clinical and allied health interventions, and monitor outcomes against each persons individual plan. Success in this role is seeing people regain function and independencesupporting a confident return home with the right services in place, or a smooth transition into permanent residential aged care when needed. Permanent, fulltime role | MondayFriday (core business hours and on call flexibility over weekends/PH) What youll be responsible for Lead safe, highquality clinical care and daily operations across the 41bed TCP service. Ensure compliance with Transition Care Program requirements, funding and reporting obligations. Embed personcentred, goaldirected reablement practice and measurable outcomes. Build a highperforming team through recruitment, coaching, capability development and effective rostering. Partner with the multidisciplinary team (nursing, allied health, medical and support services) to coordinate seamless care and discharge planning. Maintain strong governance and compliance with the Aged Care Quality Standards and relevant legislation. Manage financial performance, monitor budgets and identify efficiency opportunities. Drive continuous improvement, quality activities and readiness for audits/accreditation. What youll bring Proven leadership experience as a Service Manager (or equivalent) in aged care, subacute, rehabilitation or transitional care settings. Relevant clinical qualification and AHPRA registration (desirable). Strong working knowledge of accreditation, quality systems and the Aged Care Quality Standards. Confidence leading multidisciplinary teams and building effective relationships with hospitals and external stakeholders. Sound commercial capabilitybudget management, resource allocation and operational decisionmaking. A proactive, datainformed approach to quality improvement and change.
