Achieve Something Great Make a meaningful difference by joining our Child and Family Health Nursing teams at Bowral and Rosemeadow Community Health Centres as a Social Worker, supporting individuals, families and communities through highquality, evidencebased and consumercentred care. Working alongside child and family health nurses in collaborative, communitybased settings, you will provide holistic earlyintervention support to families during the first 2000 daysfrom conception to age fivehelping to improve health and wellbeing outcomes for children, parents and carers. This role offers fulltime work across both sites or a shared parttime arrangement, and will see you apply traumainformed practice to recognise risk, promote safety, and sensitively balance support, advocacy and mandatory reporting responsibilities. You will be supported to grow your career through professional development, supervision and training, while empowering families to feel informed, connected and confident during this critical stage of their childs development. You will play a key role in: Conducting psychosocial assessments that inform Social Work early intervention and planning. Supporting parents to navigate services, understand care pathways, and access appropriate supports such as parenting programs, early childhood services, and community resources. Providing early intervention, advocacy, and coordinated care to strengthen family functioning and address child safety concerns. Ensuring care is trauma informed, culturally safe, and aligned with best practice standards in perinatal and early childhood wellbeing. Working collaboratively with multidisciplinary teams to promote secure parentchild relationships, family resilience, and equitable access to care. You bring experience in psychosocial assessment, advocacy, and patient centred care, with a strong commitment to achieving health outcomes. You bring experience working within multidisciplinary teams, ensuring care is coordinated, responsive, and culturally respectful. You demonstrate professionalism through reflective practice, clinical supervision, ongoing professional development, and participation in quality improvement activities. You work collaboratively, engage respectfully with families, and uphold the values and ethical standards of NSW Health and the AASW Code of Ethics. You manage your workload efficiently, maintain accurate documentation, and contribute positively to team goals, service development, and student learning.
