Job Summary 360 Health + Community is the provider of the Peel Youth Mental Health Navigation and Coordination Service (PYMHNCS), an innovative service navigation model for young people 12-25 living in Peel and Serpentine/Jarrahdale and experiencing a mental health condition and one or more biopsychosocial complexities impacting their care. The PYMHNCS is a Wandjoo Gateway service partner and supports and advocates for an integrated youth mental health service across the Peel and Serpentine/Jarrahdale regions. Mental Health Clinicians provide specialist, trauma-informed mental health treatment to individuals and families impacted by complex psychosocial needs and other mental health challenges. Working within their professional scope of practice, clinicians such as psychologists, mental health nurses, social workers, counsellors and other qualified practitioners deliver evidence-informed therapeutic interventions designed to support safety, wellbeing, recovery and resilience. They will work with other members of the Peel Youth Pathways (PYP) team, to ensure young people and their families who are seeking support are able to do so in the most coordinated and effective manner possible. In this role, clinicians conduct staged and trauma-informed mental health assessments, collaborating closely with clients to develop, implement and regularly review client-led care plans. Clinicians actively participate in clinical supervision, reflective practice and professional development to ensure continual growth of therapeutic skills and maintenance of best-practice standards. They will work with other members of the Peel Youth Pathways (PYP) team, to ensure young people and their families who are seeking support are able to do so in the most coordinated and effective manner possible. The position requires a strong commitment to cultural safety, child safety, privacy and legislated information-sharing requirements. Clinicians are responsible for maintaining timely, accurate and compliant clinical documentation and data entry to support service quality, governance and continuity of care. KEY DUTIES & RESPONSIBILITIES Clinical Service Delivery Conduct initial assessments of needs and develop a Recovery Action Plan and associated risk and safety assessments. Provide one-to-one outreach support for youth with a diagnosed mental illness (or awaiting a diagnosis) enabling access and engagement with services identified in their Recovery Plan as being necessary to address their bio-psychosocial needs. Deliver specialist, evidence-informed therapeutic interventions within professional scope of practice. Conduct trauma-informed mental health assessments. Assist and guide youth with building a capacity to self-advocate or assertively communicate their needs. Communicate effectively and build positive relationships with clients, other health professionals and services, family members and carers. Follow ongoing risk assessment procedures and management plans under the supervision of the Senior Navigator/Practice Lead and/or Team Leader. Participate in internal and external stakeholder meetings as directed by the Team Leader. Keep accurate and up to date client records on the 360 Health client management system (Lumary). Facilitation of referrals and engagement with external organisations and services in support of the clients needs. Adhere to 360 Health policies and procedures. Community Engagement and Promotion Participate in promotional activities as directed by the 360 Health Marketing team, which may include visits to referral partners, community events, brainstorming sessions and others. Proactively look for opportunities to promote 360 Health in the community. Professional Standards Maintain a commitment to professional development and relevant Code of Ethics REQUIREMENTS & ATTRIBUTES Essential Full Registration as a Psychologist, Mental Health Nurse, Mental Health credentialled Social Worker, Mental Health Occupational Therapist AASW or AHPRA. High level of written communication skills particularly in the development of quality mental health care plans, case notes and report writing. Excellent verbal communication and interpersonal skills, to effectively liaise, consult and negotiate with a wide variety of people including patients, GPs, Psychiatrists, allied health staff and other stakeholders. Demonstrated ability to work effectively in a team environment and individually with minimum supervision. Demonstrated experience in and understanding the principles of confidentiality, particularly in relation to the handling of patient information. Demonstrated understanding and ability to formulate and utilise outcome measurement tools. Experience in the provision of capacity building support to clients with diagnosed mental illnesses and complex needs, and their families and carers. Demonstrated ability in developing and interpreting care/action plans to support clients in achieving agreed goals. Minimum of 2 years experience in direct service provision to young people and their families through a variety of programs and initiatives. This would include a strong understanding of how to work appropriately and effectively with young people aged 12 to 25 years. Understand recovery oriented, trauma informed, and client centred practices. Experience with client management systems and case noting. Demonstrated understanding of the issues and factors that influence the mental, physical, social, and emotional well-being of young people aged 12 to 25 years of age and their families. This would also include an awareness of the range of professionals that work within, or meet, the youth health sector. Empowering young people to participate in their own recovery and the ability to influence others to do the same.Desirable Knowledge of the Australian National Mental Health Strategy and Wandjoo Gateway. Experience working in an outreach role. Experience working with Aboriginal and Torres Strait Islander people. Self-motivated with ability to display initiative. Participation in the maintenance of a Quality Management System as part of a team. Current knowledge of Occupational Work Health and Safety and Equal Employment Opportunity principles and practices.
