The person in this position will continue to develop, facilitate, implement and evaluate management plans for patients with chronic complex health needs. The incumbent will be involved in educating Junior Medical Officers (JMOs) and GP Registrars in these areas, as well as educating patients with a focus on preventative health measures.People of Aboriginal and/or Torres Strait Islander background encouraged to apply.Candidates will need to meet the following criteria:Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).Outline your experience working in Primary Health CareCultural awareness and sensitivity to the needs of Aboriginal and Torres Strait Islander consumers, families and communities is paramount in this role.
