About the Role We are seeking an experienced and highly organised Registered Nurse (RN) to support a growing case conference program across multiple general practices. This is a patient-facing coordination role, where you will be responsible for engaging patients before and after multidisciplinary case conferences. You will play a key role in ensuring patients are informed, supported, and connected to the outcomes of their care. This is a results-driven role, where success is measured by patient engagement, consent rates, and effective communication of clinical recommendations. The position will commence at 1 day per week, with the opportunity to expand as additional clinics are onboarded. Key Responsibilities Contact patients on behalf of partner clinics to obtain informed consent prior to case conferences Clearly explain the purpose and structure of the case conference in a patient-friendly manner Following each conference, contact patients to communicate the panels recommendations Ensure all patient interactions are handled with professionalism, empathy, and confidentiality Accurately document all communications and outcomes in line with clinical and privacy requirements Liaise with internal teams and clinics to ensure smooth delivery of the program About You Registered Nurse (AHPRA registered) Previous experience in General Practice Familiar with Case Conferencing and/or Chronic Disease Management (CDM) Excellent communication skills, particularly over the phone Ability to explain clinical concepts in a clear, simple, and patient-friendly way Strong organisational skills and attention to detail Comfortable working independently in a remote environment Reliable, proactive, and outcome-focused What We Offer Fully remote role work from home Full flexibility in how you structure your day A results-focused environment that values outcomes over hours Opportunity to grow your hours as the program expands Be part of an innovative model improving patient outcomes and care coordination Supportive and collaborative team environment How to Apply Please submit your CV along with a brief cover letter outlining your experience in general practice and patient communication and understanding of case conferring .
