Social Futures is seeking a GP or Nurse Practitioner to join our dynamic Bridge AOD Program and Pillars of Support service for carers and supporters in the Singleton and Hunter areas, this role is for 4 days per month, 1 or more of those days will be based in our Singleton Clinic. Our Singleton CBD office is home to a collaborative, person-centred team, co-located with Mental Health Services two days per week to deliver integrated psychological and mental health support. We are expanding our services to include primary health care one day per week, focusing on the health needs of people who are stable on Opioid Agonist Treatment (OAT) and transitioning from their initial prescriber. About the Role Become part of a multi-disciplinary, whole-of-life regional practice. Youll provide primary care and medical oversight, while our AOD case managers deliver wrap-around psychosocial support for your patients. Together, youll help people achieve better health outcomes and long-term recovery. Experience Required A GP or Nurse Practitioner with an interest in Alcohol and Other Drugs (AOD) Already prescribing opioid pharmacotherapies or motivated to gain this accreditation (with free training and paid support) Passionate about community health, improving outcomes in regional communities, collaboration with care teams, and continuity of care If youre ready to make a difference in regional health, we would highly encourage you to apply.
