In this role, you will deliver advanced, patient-centred care, focusing on teaching patients and their carers to perform haemodialysis at home, for people living with end stage renal disease.In this role, you will:Education and Training: Training patients and carers on home haemodialysis (HHD) Fresenius 4008 machines, techniques, and troubleshooting.Support and Monitoring: Providing ongoing clinical support, often including telehealth or home visits, to manage vascular access (AVF/AVG/CVC) and assess treatment effectiveness. Advocating for patients, while supporting innovative models of care, to reduce avoidable hospital presentations.Patient Autonomy: Empowering patients to manage their own care, improving quality of life, and offering flexibility.Clinical Environment: This role is based in the hospital outpatient settings, with some travel to patient homes and other sites.Multi-disciplinary: Working and collaborating care for the patient and carers, alongside multi-disciplinary team members, for patient centred care outcomes.For a full list of role responsibilities, please review the Position DescriptionAbout YouOur ideal candidate will demonstrate:Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered NursePost graduate qualifications relevant to renal nursing, and at least 3 years fulltime equivalent clinical experience.
