Referral enables care to be implemented in the patients home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes. This position is responsible for providing specialist Social Work service to keep consumers and their carers well in the community, reducing hospital admissions and facilitating safe, early discharges from hospital. The position will ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Code of Conduct.
