The team provide specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. The role will be based at Croydon.Community Mental Health Team (Core Team) is the mainstay of the community mental health system.Core Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination. Our unique New Employee Skills & Transition (NEST) Program is designed specifically for community mental health clinicians (CMH) and is a requirement for new clinicians with two or less than two years experience working in CMH. During the first three months in the role, you will be supported to participate in the training program to enhance your understanding of the community clinician role and to deliver safe and high-quality care. With guidance from senior clinicians and managers, you''ll develop your skills in a structured and supportive environment.
