Patients who have attended an outpatient department who require inpatient admission may also be admitted via the Transit Lounge before being transferred to a ward.On the day of discharge suitable patients are transferred to the Transit Lounge to finalise their admission. The Registered Nurse is responsible for the collection of patients from the ward and ensuring the admission and final care process for a safe and efficient discharge is managed.The Transit Lounge is operated Monday to Friday 09:30 to 18:00hrs.The successful applicant will work closely with the multi-disciplinary team to deliver care and collaboration for discharge including follow up appointments, scripts, medical certificates and ensure transport.Challenge yourself.
