About the Role We are currently seeking a dedicated and experienced Care Coordinator to join our growing team in Bendigo. This role is ideal for someone who is passionate about making a difference and has strong knowledge of the NDIS framework. As a Care Coordinator, you will play a key role in managing participant supports, coordinating services, and ensuring the delivery of high-quality care aligned with NDIS standards. Key Responsibilities Coordinate and manage supports for NDIS participants Develop, implement, and review individualised care plans Conduct participant onboarding, including intake, assessments, and service setup Meet with participants face-to-face to build relationships, understand needs, and review supports Liaise with participants, families, support workers, and external providers Monitor participant progress and outcomes Maintain accurate case notes and documentation Ensure compliance with NDIS Practice Standards and organisational policies Assist with rostering and workforce coordination as required Participate in a rotating after-hours on-call roster to support staff and participants as needed About You Experience in disability services, NDIS, or care coordination Strong understanding of the NDIS and person-centred approach Excellent communication and relationship-building skills Strong organisational and time management abilities Ability to manage multiple participants and competing priorities Proficient with documentation and reporting systems Essential Requirements Qualification in Community Services, Disability, or related field (or relevant experience) Current NDIS Worker Screening Check Working with Children Check Current Drivers Licence and full comprehensive car insurance Current First Aid & CPR certification What We Offer Supportive and collaborative team environment Ongoing training and professional development Opportunity to make a meaningful impact Regular team meetings, check-ins, and collaborative support Supportive and approachable leadership team Access to mentoring and supervision to support your success in the role How to Apply If you are passionate about supporting people with disability and want to join a values-driven organisation, wed love to hear from you.
