NEBIRS provides specialist brain injury rehabilitation services across the region within a supportive multidisciplinary framework.About the RoleThis role forms part of a two-person paediatric case management service providing specialist brain injury rehabilitation for clients aged 018 years. You will work closely with another Paediatric Case Manager while receiving regular support from the adult multidisciplinary NEBIRS team through structured case conferencing and ad hoc consultation.The role includes:Community-based case managementHome and school visitsRisk assessment and care planningBuilding therapeutic relationships with children, young people and familiesFor suitable clients over 16 years of age in the northern Hunter New England LHD, access to the Transitional Living Unit supports rehabilitation goals and independence.
