Their aim is not only to improve the health outcomes of older people but also minimise the requirement to remain in hospital and to prevent readmissions once they are discharged.Lead comprehensive geriatric assessmentsin the ED to determine safe and timely pathways for older patients, ensuring decisions support discharge, shortstay admission, or escalation to acute care.Drive effective care planning and patient flowby creating tailored, personcentred plans that enable safe discharge or appropriate admission, reducing congestion and improving ED efficiency.Strengthen service navigationby identifying clinical, functional, and social needs and connecting patients with the right hospital, community, and agedcare services to maintain continuity of care.Champion early identification and escalationof geriatric syndromes such as delirium, frailty, falls risk, and functional decline working closely with ARIA and Advanced Aged Care teams to expedite assessment and reduce ED delays.Enhance delirium and BPSD managementby supporting ED clinicians with bestpractice, personcentred strategies, collaborating with families to develop PersonCentred Profiles and applying TOP 5 approaches to improve safety and minimise harm.Are You the Right Fit?We are looking for a candidate who can:Apply advanced clinical judgementduring comprehensive assessments, developing specialist management plans that optimise outcomes for older people in the ED.Brings strong knowledge of geriatric syndromes, frailty, dementia care, and falls prevention, enabling safe, evidencebased decisionmaking.Understand and can navigate inpatient and communitybased services, ensuring seamless transitions and continuity of care.Work confidently both autonomously and within a multidisciplinary team, supporting, educating, and advocating for patients, families, and caregivers.Demonstrate excellent communication, documentation, organisation, and teaching skills, contributing to safe handover practices and supporting the agedcare education needs of ASET and ED staff.Selection CriteriaTo help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.Please outline your skills and experience in providing care to older adults, particularly those with complex agerelated conditions, and describe how you ensure safe, personcentred, and culturally appropriate practice in this population.Can youdescribe a time when you completed a comprehensive assessment for an older person and used your clinical judgement to develop a management plan that significantly improved their outcome?
