Where Youll Be WorkingYoull be part of theAcute to Aged Related Care Services (AARCS)team atInverell Hospital, working within a brand-new inpatient facility. The unit has32 beds, caring for both adults and paediatrics, and youll work closely with the inpatient unit and a multidisciplinary team thats committed to safe, highquality, personcentred care for older people.About the RoleAs aRegistered Nurse - Acute to Aged Related Care Services (AARCS), youll completecomprehensive assessmentsfor people aged 65 years and over with complex health and social needs. Youll work alongside the discharge planner and multidisciplinary team to develop and implement safe, appropriate discharge plans that support older adults to transition from hospital to home or other care settings. Your role will focus on identifying risks, coordinating care, and helping prevent avoidable readmissions by ensuring older patients have the supports they need in place. Reporting directly to theNurse Unit Manager (NUM) of the Inpatient Unit, youll have access to ongoing education from theClinical Nurse Educator (CNE), flexible days and the option to increase hours in the inpatient ward.About YouYoure aRegistered Nursewho is passionate about improving outcomes for older people in the acute setting.
