Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory. About the role As the Community Health Practitioner Aged Care, you will be working within a multidisciplinary team to address the unique needs of the elders in our care and bridge the gap between our aged care and clinic services, ensuring continuity of care for each elder. This involves working with a strengths-based, culturally grounded approach to complete clinical assessments, care planning, health education, and advocate for elders within the community aged care setting. Responsibilities: Duties include, under the general supervision of an AHP Grade 3 or 4/ RN/CNC: Provision of a range of clinical services, including health assessments and care planning, all delivered with cultural sensitivity and in coordination with the DDHS multidisciplinary team. Ensure care, services and goals remain culturally centred and aligned with changing needs and preferences of each elder. Be responsive to changes in elders health, wellbeing, or environment through clinical collaboration with DDHS Primary Health services and CHSP/Aged Care allied health to identify further clinical needs and assessment. Support elders through the My Aged Care assessment process, which may include briefing assessors, involvement in assessment interviews, collaboration with assessors to develop appropriate support plans, and reviews of support plans. Provide support for elders and family members in relation to elders care needs, which may include linking clients to other related services, transfer of care and information relating to care options and costs. Provide advice and referrals for clients in accordance with clinical guidelines and protocols. Comply with DDHS Privacy and Confidentiality of Health Information Policy. To be successful you will need: Essential A Community Health Practitioner Aged Care has: Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) and current full registration with no conditions as an Aboriginal Health Practitioner under the current Australian Health Practitioners Regulation Agency; or Diploma of Nursing (Enrolled Nurse) and current full registration with no conditions as an Enrolled Nurse with the Australian Health Practitioner Regulation Agency (AHPRA); Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions and relevant management strategies. Demonstrated competence working within multi-disciplinary teams or independently under supervision, with strong organisational skills and the ability to plan, manage and prioritise tasks effectively. The ability to work in a community setting Desirable Demonstrated experience working in a Aged Care setting Knowledge of Aged Care Quality Standards and regulatory requirements in aged care. Experience using Leecare or Communicare systems. Ability to work autonomously What we offer: 6 weeks annual leave per annum 5 days paid leave during end of year shutdown (on top of annual leave).
