This temporary assignment will provide coverage for staff on meternity/parental leave. The position will ensure the provision and delivery of highquality clinical pharmacy services. In this role, you will be an active member of the multidisciplinary team, enhancing patient care processes and promoting medication safety by ensuring best practice in therapeutics and maintaining accuracy at all transitions of care, including admission, inpatient treatment, and discharge. You will also play a key role in liaising and collaborating with community pharmacists and general practitioners to support continuity of care and improve patient outcomes.The successful incumbent may require participation in pharmacy after hours and oncall services, as well as coverage of the afternoon shift on a rostered basis.
