This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. The new organisation will bring together the deep community connections, shared values and specialist expertise of both entities. Services will span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing, delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding. About the Role Direct Care Workers provide whole of client support, in order to empower clients to lead fulfilling lives according to their own choices. Providing quality, practical, efficient, and effective services to clients includes but is not limited to domestic assistance, personal care assistance, respite care and delivered meals. Services may also include support for social connection to community, family, and friends, assisting clients in pursuing their wide variety of hobbies, interests, and passions. Use of own vehicle is required. Who are we looking for? To be successful in this role, you will have a strong commitment to working from an empowerment perspective, promoting the clients independence, skill development, and self-advocacy. Your day may include (but is not limited to): Domestic Assistance Performing household tasks including vacuuming, sweeping, mopping wet areas, and cleaning bathrooms and toilets Assisting with laundry and essential ironing Supporting clients with local shopping and errands (using own vehicle) Personal Care Assisting clients with showering, bathing, dressing, grooming, toileting, and mobility Supporting clients with meals, eating, and transfers in and out of bed or vehicles Transporting clients to shopping and medical appointments (own vehicle for local travel; agency vehicle for outoftown appointments) InHome Respite and Social Support Providing inhome respite care while ensuring client safety and supervision Light household tasks during respite care Supporting clients to attend outings and social activities (e.g. coffee, lunch, parks, libraries) Encouraging social connection and relationshipbuilding through community activities and groups To be successful in this role, you will meet the following selection criteria: Minimum Certificate III in Home and Community Care, Aged Care, Disability, or Individual Support (essential). Note applicants with Certificate IV without Personal Care are not eligible. Current first aid and CPR. Available for a minimum 25 hours per week, which may include early mornings, late afternoons, and some weekends. Penalty rates apply as per the Award. Current drivers licence and registered vehicle. Yearly influenza vaccination.
