Youll work closely with prescribers and clinicians, offering pharmacy expertise that supports comprehensive care planning. A key part of your role will involve reviewing medication orders to ensure compliance with the State Poisons Act, relevant regulations and Health Department guidelines, while maintaining alignment with the Hospital Formulary. When required, you will complete documentation to initiate the supply of nonformulary medications and assess the clinical appropriateness of drug therapy by considering medical history, clinical investigations and individual patient needs.
