Deliver expert clinical care and comprehensive care planning for patients with complex needs Lead quality improvement initiatives and implement evidence-based practice Provide clinical leadership, mentorship and specialist advice to nursing staff Coordinate multidisciplinary care to reduce hospital admissions and length of stay Promote best practice standards in documentation, safety, medication management and patient dignityHow to ApplyApply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date.Essential Criteria: Current Registration as a Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA)Selection Criteria:1. Demonstrated at least three (3) years full time equivalent post registration experience in Palliative Care, Cancer Care Services, Advanced Nursing or clinical experience deemed appropriate by the employer2. Post graduate qualifications in Palliative Care, Cancer Care or Advanced Nursing3. Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning4. Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Policies and National and State Standards related to Palliative Care5. Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations6. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based service7.
