Position Title: Indigenous Outreach Worker Allan Walker Duration: Full-time, fixed term until 30 June 2026 Location: Offsite - Alan Walker Cancer Care Centre Base Salary: $72,413 to $77,931 per annum (HPCS 2.1 to 2.4) This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT). WHO ARE WE? Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory. ABOUT THE ROLE The Indigenous Outreach Worker provides extensive support to clients of Danila Dilba Health Service to address social and family needs and to ensure their access to health services in the region. This includes significant internal and external people liaison, negotiation, and the resilience to persist in an often-challenging health care environment. Our Indigenous Outreach Workers work autonomously with our clients and their families, so they need to exercise a mature and culturally appropriate manner to be able to suit the needs of our clients WHAT YOU WILL DO Maintain regular contact with clients within the scope of practice that have been identified by the Care coordination. Facilitate client referrals to specialists and other allied health services, both internal and external to DDHS, and when required, act as the liaison person on the clients behalf. Encouraging and helping Aboriginal and Torres Islander people to attend appointments with the GPs, including for Aboriginal and Torres Islander Health assessments and care planning. Encouraging and assisting the Aboriginal and Torres Strait Islander people to: attend appointments with referred specialist services and care coordinators as necessary, attend appointments for relevant diagnostic tests and/ or referrals to the primary health care prover including allied health. Collect prescribed medications from the pharmacist. Assist with planning, delivery, and evaluation of specific health program and community activities. Assist with client follow-up and recalls as directed and coordinated by the care Coordination. SELECTION CRITERIA Essential Previous experience working in the Aboriginal Health Sector. Demonstrated ability to work within and maintain strict confidentiality guidelines and policies. The ability to interact with a range of health professionals including medical practitioners and allied health professionals. Ability to work independently, and as part of a team, with an ability to plan, manage and prioritise tasks and resources to deliver outcomes within time and budget considerations. Demonstrated computer skills, especially in the use of client information systems or similar Desirable Experience working in a clinical practice or community health education program. Relevant qualifications in the industry. IN RETURN WE OFFER 6 weeks annual leave. 5 days paid leave during end-of-year shut down (additional to annual leave).
