SCHADS 6B ($55.72-$58.19 ph) + superannuation + salary packaging benefits Full time opportunity based in Fairfield VIC or Upper Mount Gravatt QLD About the opportunity The Clinical Care Consultant (CCC) plays a pivotal role in strengthening clinical decisionmaking, practice quality and responsiveness for participants with complex support needs across our NDIS, iCare and youth outofhome care programs. The CCC works within a small team of three experienced registered nurses supporting participants across the eastern coast of Australia. This collaborative model enables close peer support, shared clinical problemsolving and consistent practice, while maintaining autonomy and opportunities for leadership. It offers experienced nurses the benefits of teamwork without the isolation of a standalone advisory role. Providing clinical leadership and oversight to frontline teams, the CCC supports participants with a wide range of complex needs, including physical health conditions, acquired brain injury, intellectual disability, behaviour supports and mental health presentations, within home and community settings. Drawing on proven nursing expertise, the CCC builds workforce capability, leads the development and review of individualised complex care plans, and works closely with multidisciplinary teams to support effective assessment, risk management and escalation. A core element of the role is strengthening clinical capability across services. This includes providing individual and group based education sessions and competency assessments, and supporting teams to respond confidently, therapeutically and consistently to evolving clinical needs. The CCC works closely with specialist services and internal stakeholders to support coordinated, personcentered care that remains grounded in community practice. Rather than carrying a direct caseload, the role allows nurses to step back and influence care across multiple settings-ensuring participants receive timely, clinically sound and wellcoordinated support in their own homes and communities. For community nurses, this role offers the opportunity to apply deep clinical and systems knowledge at a broader level, shaping how care is delivered rather than delivering it alone. The CCC contributes to the development and continuous improvement of organisational clinical frameworks, governance and practice standards, translating realworld community nursing experience into sustainable, highquality care models. To be successful in this role, you will need: Bachelor''s degree qualifications in Nursing or Mental Health Nursing Current registration with the professional body relevant to qualification Strong relational skills, with the ability to build trust and work effectively with diverse stakeholders across teams, services and disciplines.
