Apply your OT skills to set goals and work with clients in the community.Where You''ll Be Working:Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.PaCH''s services are provided at the District''s hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.The Care Navigator (Integrated Care Unit) works collaboratively with clients and families, general practitioners, practice staff and other health and social services to co-ordinate access to multidisciplinary care and services for people with chronic and complex health issues. Care navigation functions are carried out in accordance with the care plan developed by the patients GP or other health professional.
