Youll play a key role in ensuring high-quality service delivery across Western NSW and supporting the professional growth of your team. Key Responsibilities Include Leading, mentoring, and supervising Care Finder staff to deliver high-quality, person-centred servicesProviding clinical expertise and guidance on intake, referrals, and complex care navigationSupporting older people to understand and access aged care services through My Aged CareStrengthening connections between health, aged care, and community servicesMonitoring service quality through audits, reviews, and continuous improvementSupporting referral demand, waitlists, reporting, and contractual requirementsPromoting the Care Finder program across community and stakeholder networksWorking collaboratively with internal teams, community partners, and service providers About You Youre a confident communicator and supportive leader who thrives in community-based work.
