This is more than a clinical role - it is a unique opportunity to shape how falls prevention services are delivered while directly improving the health and independence of older adults in our community.Where Youll Be WorkingYoull join a warm and collaborative multidisciplinary team of allied health and nursing professionals led by an experienced Service Manager.The service delivers Cardiopulmonary Rehabilitation programs across:BelmontTorontoRaymond TerraceNelson BayNewcastleThe Newcastle Health Centre serves as the home base. The team has a long and successful history delivering evidence-based programs including:Stepping OnOtago Falls PreventionHealthy and Active Online ServicesThe culture is friendly, open and supportive. With a blend of face-to-face and Virtual care delivery, there is flexibility and creativity in how services are provided.About the RoleAs a Physiotherapist or Exercise Physiologist in this new Falls Navigator position, you will have significant scope to influence and shape how the service grows. Your focus will be supporting clients who are at high risk of falls through comprehensive assessment and personalised treatment planning. This role is uniquely positioned to navigate and link clients to the most appropriate services, ensuring care is patient-centred and clinically appropriate. You will:Provide comprehensive falls risk assessmentsDevelop individualised, evidence-based treatment plansDeliver care through individual sessions, small group programs, home visits and Virtual HealthcareEstablish and strengthen referral pathways across community servicesLiaise with multidisciplinary teams to coordinate optimal careUse project planning skills to support the development and implementation of falls programsWork directly with services better suited to meet specific client needsThere is great variety in how and where you work, making this ideal for someone who enjoys connecting with people, building strong relationships and seeing tangible outcomes from their work.
