The primary function of this position is to assist patients and families with the residential placement process from the hospital through to the community setting and any other patient/family support.The Welfare Officer will have excellent interpersonal and communication skills; be able to clearly identify problems and concerns; have a wide knowledge of aged care residential processes; and advocate and negotiate with all stakeholders to achieve positive outcomes for patients and families.People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.Candidates will need to meet the following criteria:Experience and understanding of the issues relation to older people and younger people with a disability, and the complex health problems requiring aged care placement.Demonstrated ability to work independently and as part of a multidisciplinary team.Proven written and verbal communication skills with the ability to communicate across all levels with internal and external stakeholders.Need more information?1) Click here for the Position Description2) Find out more about applying for this positionFor role related queries or questions contact Bronwyn Fisher at .This is a NSW Health Category A position which requires immunisation and screening for certain diseases.
