Youll play a vital role in preventing hospital admissions and supporting early discharge, helping patients recover safely while staying connected to their community.About the TeamYoull be part of a dedicated and supportive HITH team within a broader multidisciplinary service, working alongside Community Nursing, Palliative Care, Cardiac and Respiratory Rehabilitation, Continence Services, Dietetics, Diabetes Education, Wound Management and Audiometry. Collaboration and holistic care are at the heart of what we do.About the RoleThis is a challenging and rewarding opportunity for a Registered Nurse who enjoys autonomy, variety and meaningful patient interactions.
