Help older people access the care they need, join our supportive multidisciplinary team delivering comprehensive aged care assessments across the Hunter region.Employment Type:PermanentFull TimePosition Classification:Occupational Therapist Level 1/2, Registered Nurse, Social Worker Level 1/2Remuneration:$74,317.94 - $108,943 per annum (dependant on qualifications) + Superannuation + Salary PackagingHours Per Week:38Requisition ID:REQ644692Location:Hunter RegionApplications Close: Sunday 22nd March 2026About The RoleThe NSW Health Aged Care Assessment Service (Hunter) is seeking a motivated and experienced Clinical Assessor to join our large and supportive multidisciplinary team.In this role, you will undertake comprehensive assessments of older people in both community and hospital settings to determine eligibility for commonwealth funded aged care services and develop person-centred support plans.Clinical Assessors work closely with older people and their carers to understand their goals, preferences, and support needs. You will collaborate with Geriatricians, General Practitioners, Practice Nurses, other Health Services, and Non-Government organisations to ensure timely access to appropriate care and services.The role involves working across a variety of settings including public and private hospitals, residential aged care facilities (RACFs), and community environments, supporting older people to access the care they need.Key Details:Hours: 8:00am - 4:30pm, Monday to FridayTravel: Some travel required for client assessments and trainingTraining: Comprehensive orientation including the National Training Program (approximately 6 weeks, based at Wallsend)Where Youll Be WorkingThe NSW Health Aged Care Assessment Service is a large, collaborative, and supportive team with the main office based in Wallsend.
