You will collaborate with Geriatricians, General Practitioners, Practice Nurses, other Health Services, and Non-Government organisations to ensure timely access to appropriate care and services.The role involves working across a variety of settings including public and private hospitals, residential aged care facilities (RACFs), and community environments, supporting older people to access the care they need.Key Details:Hours: 8:00am - 4:30pm, Monday to FridayTravel: Some travel required for client assessments and trainingTraining: Comprehensive orientation including the National Training Program (approximately 6 weeks, based at Wallsend)Where Youll Be WorkingThe NSW Health Aged Care Assessment Service is a large, collaborative, and supportive team with the main office based in Wallsend.
