The Wellbeing Nurse roles are based in various public schools, that have been selected to be part of the NSW Wellbeing Health In-reach Nurse (WHIN) Program. The WHIN program aims to promote individual children and families health and wellbeing by facilitating early access to health care, prevent illness and injury. Each Wellbeing Nurse works closely with school principals, school wellbeing teams, school counsellors and student support officers to support students with complex health issues, and their families/caregivers, to access to healthcare, early child health intervention and wellbeing support.This Clinical Nurse Specialist (CNS) Grade 2 role provides targeted psychosocial and primary health assessments, and short-term healthcare coordination and navigation, to support school students attending designated schools to access various medical, health and social support services that are important to their health, wellbeing and lifelong condition management - to enable children and young peoples continued participation in school-age education.The Wellbeing Nurse is responsible for engaging and working in partnership with children, young people and their parents/caregivers, using a strengths-based framework to build rapport with clients. The role applies specialist, advanced practice nursing skills and experience in working with primary and high school age students to identify healthcare and wellbeing needs, and to coordinate care. The role takes a lead role in navigating care by organising referrals, advocating for access to services, coordinating care across multiple agencies, health services and community groups. When required, the role may include assistance with arranging and accessing primary and specialist health care appointments for clients.People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.Candidates will need to meet the following criteria:Current Registered Nurse Division 1 National Registration (AHPRA) with a minimum of 3 years post registration clinical experience working with school-age children and young people, a post registration qualification related to the specialty area of Adolescent and Young Adult Health, Child, and Family Health Nursing, or Paediatrics, or such other qualifications or relevant equivalent experience. Current NSW driver''s licence and willingness to use for work purposes.Experience and/or demonstrated ability to work within a community health, school and/or health interagency contexts, to develop relationships supporting integrated, coordinated and client-centric care for school-age children, young people and families/carers in a school context.
