As a Health Clinician Level 3, you will play a pivotal role in providing trauma-informed, culturally safe,and child-centred care.In this role, you will:Conduct comprehensive health assessments and develop individualised care plans tailored to the needs of children and young people in OOHCWork closely with carers, caseworkers, and families to ensure coordinated, holistic, and culturally respectful careAdvocate for children and young people to access the health services they need, including both public and private sector supportCollaborate with a wide network of health professionals, community organisations, and Aboriginal and Torres Strait Islander services to strengthen support pathwaysContribute your expertise to a team that values innovation, professional development, and evidence-based practiceThis position provides the opportunity to make a meaningful difference in the lives of vulnerable children and young people while applying your professional skills in a culturally safe and supportive environment. You will gain valuable experience across a broad health network and have access to ongoing training and development to support your career growth.Key ResponsibilitiesConduct clinical triage of new referrals to the OOHC Health Pathway, identifying health needs and recommending appropriate services or interventionsComplete or coordinate comprehensive health assessments for children and young people, aligned with NSW Clinical Practice Guidelines and within your scope of practiceDevelop, implement, and review individualised health plans in collaboration with children, young people, carers, DCJ staff, NGOs, and the multidisciplinary teamAdvocate for children and young people to access culturally safe and trauma-informed health services, both in public and private sectorsLiaise and build strong relationships with key stakeholders, including Aboriginal and Torres Strait Islander services, to ensure coordinated and culturally responsive careMonitor and follow up on clinical recommendations, ensuring timely access to appointments, treatments, and interventionsProvide direct clinical support in OOHC-specific clinics where relevantIncorporate considerations such as living arrangements, court proceedings, placement changes, and leaving care into care planning and service deliveryModel the HNE Health Values Charter, NSW Health Code of Conduct, and cultural respect in accordance with Closing the Gap strategiesMaintain accurate records, contribute to data reporting, and support continuous quality improvement initiativesWho You Are and Your Community ConnectionWe are looking for an Aboriginal and/or Torres Strait Islander Health Clinician who is deeply committed to supporting the health and wellbeing of children and young people in out-of-home care.
