The position is fully remote, to be based anywhere in Queensland. This is a fixed-term position for one year. About the role: This role is new for Soldier On and requires knowledge that draws on the full breadth of social work practice assessment, coordination, systemic thinking, and relationship building, within a focused and specialised team. You will not be working in isolation. You will be part of a multidisciplinary group that includes clinical psychologists, psychologists, group facilitators, peer supportworkersand program staff, all working toward the same goal. As Intake and Case Management Coordinator, you are the first point of contact for veterans and their families registering with SoldierOnPsychology Services in Queensland. Your role is to conduct structured, trauma-informed intakes via Telehealth, assessing need across multiple life domains including mental health, physical health, housing, employment, social connection, and family, to develop a clear picture of where each person sits and what they need. From that assessment, you will coordinate warm referrals to the right support: internal programs and clinicians, DVA-funded services, community mental health, peer support, employment and education services, or other relevant organisations. Where ongoing coordination is needed, you willmaintaincontact to ensure clientsremainconnected and supported across those domains. This role requires someone who understands that veterans and their families rarely present with a single need. The work calls for a practitioner who can hold complexity, work across systems, and build trust quickly with clients and with the services you connect them to. Hours:Part-time (0.5 FTE), 19 hours per week Employment term:Fixed term appointment for one year. The position is fully remote position based in Queensland, flexible across days to suit your schedule. Key Responsibilities: Conduct structured, trauma-informed intakes via Telehealth, assessing need across mental health, social, functional, and family domains Develop a clear formulation of each client''s needs and identify the most appropriate internal and external referral pathways Coordinate warm referrals to internal programs and clinicians, DVA-funded services, community mental health, peer support, employment services, and other relevant supports Maintain follow-up contact with clients to confirm connection to services and provide further coordination where needs change or referrals have not been taken up Build and maintain a current working knowledge of the veteran services landscape in Queensland across health, mental health, housing, employment, and social domains Conduct compassionate, person-centred risk assessments when relevant and escalate to the clinical team appropriately Maintain accurate and timely clinical and administrative records using platforms including Halaxy, Dynamics, Qualtrics, and NovoPsych Respond promptly to enquiries and ensure clients receive clear, helpful direction Contribute to data collection and program evaluation processes Regularly update policies and procedures to ensure knowledge and processes represent best practice Maintain confidentiality in accordance with professional obligations and organisational policy Work closely with the Intake Manager and clinical team, seeking consultation when navigating complex presentations Identify gaps or concerns in systems and processes and raise these with the Health and Wellbeing Director and update accordingly About you: You are an experienced social worker, registered or eligible for registration with AASW, with a background in case management, intake, or coordination roles. You are confident assessing need across multiple life domains and know how to hold complexity without losing sight of the person in front of you. You have a strong working knowledge of the veteran services landscape in Queensland. You understand the DVA system, know the key services and organisationsoperatingin this space, and can navigate referral pathways efficiently. You are equally comfortable working independently,managing your own caseload, prioritising competing demands, and making sound judgements without on-site support, and working as part of a team. You bring professionalism and warmth in equal measure, and you understand that the first conversation a veteran has with a service oftendetermineswhether they come back. Experience withHalaxy, Dynamics, Qualtrics,NovoPsych, or similar platforms is an advantage. Familiarity with data collection, reporting, and outcome measurement will also be well regarded.
