We''re looking for a Registered Nurse (RN) who is a confident communicator and organised self-starter to help Santa Clara Home Care build its local presence, build our workforce, and deliver exceptional client care in Wollongong and surrounding areas. Working under the guidance of our Care Manager based at our Head Office in Mascot NSW, you''ll have the autonomy to shape service delivery while being supported by an experienced leadership team. About the Role Work from Home Duties Client Care, Communication & Advocacy coordinate day-to-day client needs, update care plans, complete documentation, conduct follow-up calls, handle client queries, communicate with referral partners, and guide clients on package use. Workforce Support & Operational Oversight screen, interview, onboard, and support staff remotely, monitor training compliance, organise rosters, coordinate daily operations and urgent coverage, and escalate risks as needed. Compliance, Quality & Documentation maintain accurate records, complete incident reporting, uphold quality standards, use internal systems securely, and support continuous improvement work. On the road duties Client Care, Advocacy & Case Support conduct in-home assessments, provide nursing care, complete home safety checks, deliver occasional personal care, and build relationships through face-to-face contact with clients, families, carers, and referrers. Workforce Support & Supervision conduct in-person interviews, deliver local induction, and complete supervision visits and support carers. Regional Presence & Service Development represent SCHC locally, introduce services to hospitals and community partners, distribute information, identify local needs, support community-based initiatives, and contribute to service development activities in the region. About you Experience & Qualifications Registration & Compliance Knowledge Essential - Current AHPRA-registered RN Desirable - Knowledge of aged care legislation, in-home care models, Quality Standards, SIRS, and compliance processes, including incident reporting and secure system use. Community-Based Clinical Experience Desirable - Experience in community-based roles such as aged care, disability care, community nursing, dementia care, geriatric nursing, or palliative care, with the ability to work independently in clients'' homes. Workforce Coordination & Essential Requirements Desirable - Skilled in recruitment activities (screening, interviewing, onboarding), rostering oversight, and supporting care workers through training and supervision. Essential - Holds essential requirements including police clearance, driver''s licence, reliable vehicle, and digital literacy. Personality Traits Strong Communicator & Relationship Builder Able to build trust quickly over the phone and face-to-face with clients, families, carers, hospitals, GPs, support coordinators, care finders, and community partners. Empathetic and client-centred, comfortable providing advocacy, guidance, and occasional hands-on personal or nursing care when required. Organised, Detail-Oriented & Reliable Able to coordinate rosters, documentation, follow-ups, and incident reporting with accuracy and attention to detail. Autonomous, Adaptable & Proactive Confident working independently in the field and collaborating remotely with the Santa Clara Home Care Head Office, maintaining professionalism and initiative. Able to identify community needs, support service improvement, and effectively represent SCHC as the local presence in the region.
