About the Opportunity Our Community Allied Health Services deliver care across home, community and group program settings, supporting people to remain safe, independent and well in their own homes wherever possible. This role is ideal for an appropriate qualified Dietitian who thrives on a diverse, interesting and varied clinical caseload in both home and community settings, and who enjoy working in a multi-disciplinary team context. The team has a strong culture of learning and development with strong clinical supervision and support. The roles include working with clients across: Acute care needs. Chronic, progressive and neurological conditions. Age-related functional decline. End-of-life and palliative care in community settings. In these roles you will: Provide evidence-led nutrition assessment and intervention across home, community and group program settings in compliance with the Dietitians Code of Professional Conduct and Statement of Ethical Practice. Work collaboratively with multidisciplinary colleagues and allied health assistants to deliver timely, client-focused care. Support group education initiatives delivered at community centres as part of broader allied health programs. Contribute to quality improvement, research and service development activities within Community Allied Health. For more information about this role, please view the Position Description. About You We are looking for someone who has: Bachelor or postgraduate degree in Nutrition and Dietetics that provides for full membership of the Dietitians Australia and a current NSW driver licence. Clinical experience providing clinical nutrition services to clients with chronic, neurological, progressive, age-related conditions or end of life care in community settings. Strong assessment, intervention, and group facilitation skills. Ability to integrate self-management and evidence-based practice into clinical care. Excellent communication, time management, and organisational skills. Capacity to work independently and collaboratively within a multidisciplinary team.
