Part-time (0.8 EFT) Fixed-term until 30 April 2030 Generous salary packaging and flexible working arrangements Based in Seymour Join Our Team at Omnia Community HealthFrom 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health have merged to form a new, unified not-for-profit community health organisation, Omnia Community Health. This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. The new organisation will bring together the deep community connections, shared values and specialist expertise of both entities. Services will span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing, delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding. About the Role The Early Years Nurse Health Care Coordinator provides high quality, clinic based nursing care to support the health and wellbeing of infants, young children and their families. Based primarily at the Seymour Family and Childrens Centre, the role also requires flexibility to work across other Omnia sites when needed. The position focuses on early identification and intervention of health and developmental concerns, conducting comprehensive assessments, monitoring child development, and delivering evidence informed support and education to families to strengthen confidence and improve health concerns affecting vulnerable children. The role will include but not limited to: Working collaboratively with staff of Kids First Australia, Seymour College, Mitchell Shire Council Maternal and Child Health staff, Omnia Allied Health staff, Colman Education Foundation, and parents to undertake inclusion mapping. Providing direct support to children and their families in response to identified health needs including supporting and coordination of referral pathways and processes. Undertaking assessments, planning, delivery and evaluation of nursing interventions. Developing individualised treatment plans that address the specific needs and goals of each child and family. Working to identify and implement strategies with families that might appear difficult to engage with. Applying clinical skills to identify children who may be experiencing developmental delays, facilitate timely referral to appropriate support services, and recognise individuals who are not engaging with recommended interventions. Providing information, advice and support to families on health-related matters. Working collaboratively with kindergarten and school staff, to identify and develop strategies and programs to be delivered to address broader independent living, health and wellbeing issues and challenges faced by children. Meeting annual performance targets. To be successful in this role, you will have met the following selection criteria applicants are asked to address the following criteria in their application: Excellent interpersonal and communication skills, with the ability to collaborate effectively with families, colleagues and external partners while maintaining professional boundaries. Strong clinical nursing knowledge and skills, with a commitment to maintaining professional development and delivering highquality, personcentred care. Demonstrated ability to work collaboratively within multidisciplinary teams and contribute to integrated, interprofessional models of care. Welldeveloped organisational, planning and timemanagement skills to meet service priorities, performance targets and stakeholder requirements. Ability to think critically, solve problems creatively and adapt to changing service and community needs. Sound understanding of clinical governance, health equity, safety standards and continuous quality improvement. Good computer literacy, including Microsoft Office and clinical data systems, with accurate documentation and reporting skills. Desirable Relevant postgraduate qualifications in paediatrics, emergency, or primary care Relevant Certificates or additional training related to the role Current nurse immuniser qualification Demonstrated clinical experience in any of the following areas: case management, emergency, paediatric, or primary care nursing, including comprehensive assessment, care coordination, and supporting patient engagement with recommended services. Qualifications and Registration Current AHPRA registration as Registered Nurse Current First aid and CPR Certificate Minimum of four years clinical nursing experience, preferably in a relevant specialty Demonstrated knowledge of the Nursing and Midwifery board of Australias, codes, guidelines decision-making framework pertaining to scope of practice Fully vaccinated for COVID-19 Current National Criminal History Check, NDIS Worker Screening Check and valid Working with Children Check Must hold current Victorian Drivers Licence.
