About the Opportunity In consultation with the Clinical Nurse Manager, the Community Health team plans, implements and evaluates community nursing care (in the area of school health and community health) and relevant primary health programs for the communities serviced by the Primary Health Service. To be successful in this role, you will have Ability to apply knowledge and skills in the area of clinical expertise (including ability to plan, develop, implement and evaluate programmes at an individual and group level) Knowledge and understanding of the principles of Primary Health Care Effective communication (written and verbal) and interpersonal skills Ability to work independently and in a multidisciplinary team Knowledge and understanding of current Aboriginal health issues and various community resources that may be utilised to address these issues.
