This role is all about local leadership and direct connection. Community Engagement: Build and maintain strong relationships with local partners, health services, and resources to support client-centered care. Supervise and support a field-based care team, including performance management, and collaboration with internal leadership. Coordination & Travel: Manage documentation and communication tasks while traveling within the community and between client homes. The Clinical Lead will live and work in their local community, providing direct client care while building and overseeing a multidisciplinary team (e.g., Supported by our client, theyll play a key role in expanding services and fostering trusted relationships across both formal and informal care networks. This field-based role offers flexibility to work from home when not in the community. Must also do some hands-on clinical field work as they build the team.
