Job Description

This advanced clinical social work role functions within interprofessional healthcare teams to deliver holistic, patient-centered care for individuals with complex medical, psychosocial, environmental, and functional needs. It demands strong clinical judgment, critical thinking, and the integration of biopsychosocial assessments into collaborative care planning. The Social Worker III provides direct and indirect individual and group services in both in-person and virtual settings, working alongside PT, SLP, Rec T, SW, and TA. Responsibilities include maintaining clinical records, offering rehabilitation consultation, and mentoring students and new staff. This role delivers specialized interventions such as psychosocial assessments, therapeutic support, and care coordination, while also consulting with patients, families, and the care team. The position includes student supervision, team mentorship, and participation in clinical education and capacity-building. At the system level, the Social Worker III contributes to program development, Connect Care optimization for charting and service delivery, service planning, quality improvement, and applied research or evaluation, as directed by leadership. The role demonstrates leadership in ethically complex situations, supports trauma-informed and culturally responsive care, and fosters interdisciplinary collaboration. The Social Worker III addresses social determinants of health through interventions like crisis response, brief counseling, safety planning, and support with advance care planning and substitute decision-making. These services aim to enhance patient and caregiver well-being, ensure continuity of care, and support rehabilitation readiness. As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s). Master''s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). A minimum of 3-5 years'' experience as a rehabilitation clinician working in the community. Experience in providing therapy and psychosocial interventions is required, along with authorization to perform restricted psychosocial activities. Training in CBT, DBT, group psychotherapy, family therapy, or other recognized modalities is essential. Strong clinical expertise in pediatric and adult rehabilitation is necessary. The ideal candidate thrives in a dynamic environment, demonstrating adaptability, critical thinking, and sound decision-making both independently and within a team. Excellent communication, assessment, counseling, and organizational skills are required, as is the ability to build therapeutic and collegial relationships and manage sensitive situations. Responsibilities include maintaining clinical records, offering consultation, and mentoring staff or students. A commitment to constructive feedback and a strong customer service orientation are key. Knowledge of community rehab services, social determinants of health, and proficiency with digital tools (e.g., EMRs like Connect Care, virtual platforms, email, word processing) is expected. The role involves moderate physical activity (lifting 20-50 lbs.) and extended periods of seated work with technology for documentation and virtual care. Experience with Connect Care. HealthChange Methodology training and experience in virtual and group service delivery is considered an asset.
