We take an active role in working with our home operator partners to promote resident health, staff education, and medication system quality and efficiency.Whether you do your best work in a dispensary, in a clinical setting, in an office, or on the road, we have an opportunity for you.Position SummaryThe Clinical Consultant Pharmacist is a member of the multidisciplinary healthcare team supporting appropriate medication use for Residents in Long-term Care Homes and Retirement Communities. The Consultant Pharmacist works with the healthcare team to review medication use to identify medication-related problems and to achieve desired Resident outcomes. The Pharmacist will provide drug information, be involved in educational activities for healthcare staff and Residents, regularly attend medication management meetings and promote Resident safety.This position requires regular travel throughout the Greater Toronto Area (GTA) and surrounding area.Your Day To DayAct as aliaisonbetweenlong term care homesand retirement communitiesand CareRx Pharmacyto ensure optimal Resident care, Pharmacy Services, and Clinical supportBuild and maintain relationships within the community to ensure ongoing collaboration and communication between home / communityleadership teams and pharmacy teams to develop / maintain strong partnershipsComplete medication reviews in currently serviced retirementcommunities and long term care homesserviced by CareRx PharmacyComplete all required documentation and billing processes as required by MOHLTC, Ontario College of Pharmacists (OCP), and other federal and provincial standards and regulationsProvide education to Residents and staff on various health-related topics including theMedsCheckprogram and attend community-based wellness fairsand other eventsas requiredConducteducation sessions withinLong Term Care Homes and Retirement Communities withinscope of practice, as applicable (e.g. policy and procedure, web portal,eMAR, influenza protocol, etc.)Support CareRx Pharmacy policies and procedures within retirementcommunities and long term care homesCompletemedication audits on-site in LTC Homes and Retirement Communitiesas requiredEnsure regular on-site visits are made to all assigned clientsAttend / participate in annual professional conferences and trade shows, when applicableParticipate in the development and implementation of industry-leading clinical pharmacy service, when applicableBe an education, policy and pharmacy initiative resourceBe available for special clinical team projects as requiredComplete / collaborate in the develop of medication audit follow-up improvement plans when applicableWorks collaboratively with other team members to support applicable homes and communitiesParticipate in pharmacy-related transition processes (e.gnew client transitions,eMARtransitions, etc.)Help with pharmacy operations / drug distribution when requiredAttend and actively participate in clinical team meetingsQualifications / Experience :Travel to LTC homes and retirement communitiesInsured and currently licensed in good standing with the Ontario College of Pharmacists (Part A)Proven ability to foster strong partnerships with other members of the health care teamEffective organization, planning, and time management skillsKnowledge of Kroll Windows applicationand Microsoft Office programsCompleteunderstanding ofprovincial regulations governing LTC homes and Retirement CommunitiesMust be familiar with the provincial standards of practice established for LTC pharmaciesExcellent customer service and interpersonal skills, oral and written communication skillsand be able to interact with persons of varying levels of professional expertiseSkilled in critical thought processes and be able to prioritize appropriatelyMust demonstrate initiative and creativitySelf-directed requiring minimal supervision and able to work independentlyMotivated to achieve results that reflect a high standard of qualityGood listening skills and be adaptable to changes in job environmentDependable and responsible in the execution of job dutiesAdaptable to changes within internal work structuresStrong presentation and teaching skillsMust possess a valid drivers licensePreferred :1+ year ofclinical andgeriatric pharmacy experiencePrevious LTC pharmacy and / or hospitalpharmacyexperienceApplication ProcessCareRx welcomes and encourages applications from people with disabilities.
